(Richmond, IN)--An OSHA report stops short of directly blaming corroded grates for a Richmond man’s death earlier this year, but does point out six violations at the Richmond Sanitary District.  Employee Danny Caldwell died in early July at the plant on Liberty Avenue.  The initial report then was that he had fallen into a tank.  The newly released OSHA report claims that metal grating for walking at the tank was corroded to the point of potential failure.  The report also claims the grates were not inspected.  The city has about three weeks to fix the problem or be fined.
